Admissions
Process:
1. We will fill out an Intake
Referral form on the patient.
2. We will contact your primary
physician to get the order for Homecare Services.
3. Once the order is received we
will send out a nurse to fill out an Admission Packet,
this will take approximately 1
1/2 hours.
4. You will be contacted directly
after that by our Staff Coordinator to schedule the
medical personnel that you
need.
